Still food in there after twelve hours

Eighty percent of adolescents on a GLP-1 had solid food in the stomach after fasting overnight before surgery. Twenty patients, one hospital, no aspirations.

Fasting instructions before surgery assume the stomach empties. In these patients it did not. That is worth knowing before a procedure, and it is worth reading carefully, because the numbers are small and the group is young — the same care any safety signal deserves.

What they did

Sixty-seven adolescents at one hospital were scanned with gastric ultrasound before surgery. [1] Twenty were on a GLP-1. Twenty-seven were at risk of slow emptying for other reasons. Twenty were healthy controls.

Everyone followed standard rules: eight hours without solids, one hour without clear liquids. In practice they fasted longer. Median 13 hours for solids, interquartile range 12 to 14.

What they found

Solids were present in 16 of the 20 GLP-1 patients. That is 80%, after more than twelve hours without food.

The at-risk group came in at 17 of 27, or 63%. Healthy controls: 1 of 20, or 5%. The difference held after adjusting for age, body mass index, sex, race and fasting time, unadjusted p < 0.001 and adjusted p < 0.02.

Twenty people

Sixteen of twenty. Move one patient and the figure shifts five points. The result is consistent with what larger adult studies of gastric emptying have reported, and on its own it is a small single-center cohort.

Take the direction seriously and the precision lightly. That is the same reading a thin margin in a surgical database earns.

The one thing to do

Tell the anesthesiologist. Before any procedure with sedation, say that you are on a GLP-1 and say when the last dose was.

That is free and it changes what the team does — longer fasting, an ultrasound check, or a different airway plan. It is also the sort of thing that never comes up in a checkout flow, along with what is actually in the vial and what arrives when somebody orders one.

Frequently asked

Should I stop my GLP-1 before surgery?
That is a decision for the team doing the procedure. What this study supports is telling them you take one, and when your last dose was.
Does this apply to adults?
The 80% figure does not. Everyone in this study was between 10 and 18 years old, median age 14.8. The mechanism is the same one described in adults, but this number was not measured in them.
Did anyone inhale stomach contents?
No, and the study did not measure that. It looked at whether food was still in the stomach on ultrasound. Twenty patients could not detect a rare event.
How long had they fasted?
A median of 13 hours for solids, interquartile range 12 to 14 — well past the 8 hours standard guidelines require.
