The heart got smaller with the rest of you

A year of tirzepatide reduced left ventricular mass and the fat around the heart. The change tracked body weight, which is the part that decides what it means.

Obesity changes the shape of the heart. The left ventricle thickens, fat accumulates around the outside of it, and the chamber stiffens. This substudy put people in a scanner before and after a year on tirzepatide to see whether any of that reverses — which is a different question from what the tirzepatide premium buys on a price list, and a more interesting one.

What was measured

A total of 175 patients with obesity-related heart failure with preserved ejection fraction from the parent trial underwent cardiac magnetic resonance imaging at baseline, on tirzepatide titrated from 2.5 mg to a maximum of 15 mg weekly or on matching placebo. [1] Of those, 106 completed a second scan with image quality good enough to analyze — 50 in the treated arm and 56 on placebo. The prespecified primary endpoint was the between-group change in left ventricular mass. That gap between 175 scanned and 106 analyzed is worth registering before any of the numbers.

What changed

Left ventricular mass decreased by 11 g in the treated group once corrected for placebo, 95% CI −19 to −4, P=0.004. Paracardiac adipose tissue — the fat sitting on and around the heart — decreased by 45 mL, 95% CI −69 to −22, P<0.001. The change in left ventricular mass also correlated with changes in chamber and left atrial volumes, P<0.03 for all, which is the internally consistent picture you would want if the finding is real.

A surrogate is not an outcome

Nothing here is an event. Left ventricular mass is a measurement; hospitalization is an outcome. The authors say these physiologic changes "may contribute" to the reduction in heart failure events seen in the main trial, which is the correct hedge and is not the same as demonstrating it. Surrogates have a long record of moving in the right direction while outcomes do not follow, and accepting one for the other is the same habit as accepting a combination result without the comparison that would explain it.

The parent trial did measure events, and what those are worth against what they cost is a separate question. This page is only about the pictures.

What a year of it costs

The scans were taken 52 weeks apart, so the exposure being priced is a full year at a dose climbing toward 15 mg. Sellers here publish a median of $249 a month for tirzepatide by injection, read September 2026 — about $2,988 across the year if the rate holds and the dose does not change the price, which are two conditions worth checking in the cost tool before assuming either.

No seller sells a smaller left ventricle, and none of this appears on a product page. It is worth knowing anyway, because it is the mechanism underneath the claims that do get made. A structural change that parallels weight loss is a reason to expect the benefit to arrive with the weight and to leave with it, which puts a year of scans on the same side of the ledger as everything else that reverses on stopping.
