Everything on this site is about what a seller charges, so it is worth watching what actually moves volume in this market, and it is not the number on the page. It is whether a third party will pay it — the same gap between a published figure and what a person ends up paying that runs through every price this desk records.
What changed, and when
Two things happened in March 2024. The FDA added an indication to Wegovy: reducing cardiovascular events in adults with overweight or obesity and established cardiovascular disease. And Medicare Part D began covering it for that indication. [1]
Medicare is not permitted to cover a drug for weight loss as such. Attaching a cardiovascular outcome to the label is what made coverage possible, which is why the label change and the coverage change arrived together.
What happened to volume
National prescription data from September 2023 to September 2024 show total semaglutide use rising, with Wegovy up by 136.4% relative to the period before the change — the largest relative increase of any semaglutide product.
Through Medicare Part D, Wegovy rose by 598.1%. A segmented regression attributes an estimated 592,624 additional fills to the policy change by September 2024.
Who this did not help
Anyone without established cardiovascular disease. The coverage follows the indication, and the indication is narrow.
Most people who arrive at this site are not covered for any of this and are paying cash to a telehealth seller, which is the reason the site exists. A policy that moved half a million fills changed nothing about what those buyers pay.
The part worth remembering
That demand for these drugs is not price-limited so much as coverage-limited. The list price was the same in April as in February; six times as many Medicare prescriptions got filled anyway.
It also sets the pattern for what comes next. Each new outcome indication is a potential coverage unlock for the group that has it, and a non-event for everyone else — worth holding next to what preventing one of those events is actually worth.
